This form must be sent with either:

• Application for additional Business e-services AFS2
• Business e-services application form AFS1

Existing Business e-services customers

If your organisation is already using Business e-services, a responsible person can request a new deputy responsible person account by:

• filling in form AFS5a and sending it to us by post
