This form must be lodged with either your:

• Business e-services application form AFS1
• Application for additional services AFS2

Existing Business e-services customers

If your organisation is already using Business e-services, the responsible person or a deputy responsible person can request a new administrator account by:

• filling in form AFS4a and sending it to us by post
