Step 1
If you want to locate the nearest Enquiry Centre, go to Step 2.
If you want to book an appointment at the Enquiry Centre for the customer or send the case to the National Payments Team, go to Step 4.
Step 2
Access Microsoft Excel on the toolbar

- select 바카라 사이트˜xmls forms바카라 사이트™ from the options available
- select the 바카라 사이트˜National바카라 사이트™ tab on the 바카라 사이트˜Form template바카라 사이트™ menu
- select 바카라 사이트˜Enquiry Centre Appointment form바카라 사이트™ once
- select 바카라 사이트˜OK바카라 사이트™. You바카라 사이트™ll be taken to the 바카라 사이트˜Enquiry Centre Location바카라 사이트™ screen
- enter the customer바카라 사이트™s postcode to find the nearest Enquiry Centre for the customer
- enter either the 바카라 사이트˜town/city바카라 사이트™ or 바카라 사이트˜county/region바카라 사이트™
- the nearest Enquiry Centre will be displayed
- the Enquiry Centre opening hours will be displayed
- go to Step 3.
Step 3
If the message 바카라 사이트˜Refer all Tax Credit requests to National Payments Team바카라 사이트™ displays, the Enquiry Centre doesn바카라 사이트™t have the facility to provide payment to the customer.
If the message 바카라 사이트˜Refer all Tax Credit requests to National Payments Team바카라 사이트™ doesn바카라 사이트™t display, the Enquiry Centre has the facility to provide a payment, should that be necessary.
Step 4
Access Microsoft Excel on the toolbar
- select 바카라 사이트˜SEES바카라 사이트™ from the options available
- select 바카라 사이트˜xmls forms바카라 사이트™ from the options available
- select the 바카라 사이트˜National바카라 사이트™ tab on the 바카라 사이트˜Form template바카라 사이트™ menu
- select 바카라 사이트˜Enquiry Centre Appointment form바카라 사이트™ once
- select 바카라 사이트˜OK바카라 사이트™. You바카라 사이트™ll be taken to the 바카라 사이트˜Enquiry Centre Location바카라 사이트™ screen
- select your 바카라 사이트˜town/city바카라 사이트™ or 바카라 사이트˜county/region바카라 사이트™ from the options available
Note: If you need to refer the case to the National Payments Team, enter 바카라 사이트˜F2F바카라 사이트™ in the 바카라 사이트˜town or city바카라 사이트™ field.
- select 바카라 사이트˜OK바카라 사이트™. You will be taken to the 바카라 사이트˜Select Data Capture바카라 사이트™ screen
- select 바카라 사이트˜None of the above바카라 사이트™. You바카라 사이트™ll be taken to the 바카라 사이트˜Enquiry Centre Appointments바카라 사이트™ screen
- enter a response for every question shown
Note: If you바카라 사이트™re referring the case to the National Payments Team for a hardship payment, enter 바카라 사이트˜Hardship Payment Request바카라 사이트™ in the 바카라 사이트˜Nature of Query/Reason for Referral바카라 사이트™ field.
Note: The computer will display a prompt if you attempt to send the form without completing all the mandatory fields.
