The case of a 53 years old patient, immunocompromised by HIV, hospitalized with a prolonged fever
Abstract
Vaginal tuberculosis is exceptional and under-diagnosed in this region. The authors report the case of a patient of 53 years old, immunocompromised by HIV, hospitalized in the service for poor general condition in a context of prolonged fever. The interview and physical examination had found the following: chronic cougher in the entourage, multiple sexual partners and persistent leucorrhea. Mycobacterium tuberculosis was found in leucorrhea at direct microscopic examination. HIV serology was positive for HIV-1; the rate of CD4 cell count was 22 cells/ÎĽL. The diagnosis of vaginal tuberculosis in severe immunocompromised HIV was selected. TB treatment was established and antiretroviral therapy started two weeks later. The outcome was quickly favorable after 6 months of treatment. The patient was declared cured of TB. So, in a patient immunosuppressed HIV, any persistent purulent discharge even vaginal, must do search TB.
